Abstract

Class II malocclusion is one of the most frequent treatment problems facing orthodontists, representing almost one third of all malocclusions. A link between Class II malocclusion and reduced pharyngeal airway dimensions has been reported in the literature. Several treatment options have been attempted to correct this malocclusion, one of those options is to distalize the maxillary posterior teeth without extraction treatment. In 2004 a new appliance was introduced by Luis Carrière carrying his name, called the Carrière Motion appliance. However, so far no RCT in the orthodontic literature evaluated the effect of this appliance on the dimensions of the airway.
The aim of this study was to evaluate the effect of this appliance on the upper airway by CBCT images in treatment of post-pubertal patiets with dental class II malocclusion. A comparative study was conducted consisting of 18 participants with age range from 18-30 years old. The treatment effects were evaluated radiographically using pre and post-distalization CBCT images after Class I molar relation has been reached. A three dimentional analysis was performed to study the effects of distalization on the upper airway. The collected data were then statistically analyzed for treatment changes between pre and post-distalization.

On the basis of the results obtained from the present study the following conclusion could be drawn:
1. Distalization with Carrière Motion appliance had a positive effect on the upper airway.
2. Total airway volume (upper airway) as well as the MCA was increased significantly as shown by statistical analysis.
3. Most of this increase in the total airway volume is at the oropharyngeal airway area and hypopharyngeal airway compared to the nasopharyngeal airway
