Abstract

The current study was performed to evaluate the efficacy of nano curcumin gel 2% versus nano doxycycline gel (SDD 20 mg/ml)in the treatment of patients who were diagnosed with stage II grade B periodontitis. Thirty sites were selected and randomly assigned to two groups: • group I: (15 sites) was treated by nano curcumin gel 2%. • group II: (15 sites) was treated by nano doxycycline gel 20 mg/ml. The gel application was done three times : after scaling and root planing immediately, two weeks and four weeks after first one . comparison between the two groups was assessed by: 1- Clinical assessment Each patient in both groups was clinically evaluated at four times follow up: 1- Zero-time evaluation (before starting phase I therapy). 2- Baseline evaluation (one month after finishing phase I therapy). 3- At three months after baseline evaluation. 4- Six months after baseline evaluation. The following clinical parameters were evaluated: • Probing Pocket Depth (PPD). • Clinical attachment level (CAL). • Bleeding on probing (BOP). The prefabricated acrylic stent and a manual (UNC-15) probe were used to standardize the PD and CAL. For, BOP a manual periodontal probe was gently used to stimulate the tissue at the base of the periodontal pocket. 2-Immunological assessment: The immunological assessment of IL-6 in the GCF of study sites was done 3 times during the study: 1- Zero-time evaluation before starting phase I therapy. 2- Three months after baseline evaluation. 3- Six months after baseline evaluation. A standardized collection paper point was gently inserted to the bottom of the sulcus for 30 seconds. The papers contaminated with blood were discarded. Paper points were placed in a sterile microcentrifuge tubes containing 250 μL of phosphate-buffered saline and stored at − 80 °C. Concentrations of IL-6 in samples of both groups was determined by ELISA. Results of our study showed the following: There was a highly statistically significant reduction (p ≤0.05) regarding to PPD, CAL, BOP and level of IL-6 in the two groups from zero time to 6 months follow up periods indicating that all two treatment modalities individually were effective in reducing periodontal inflammation through reduction of BOP, PD with clinical attachment gain. However, when comparing the two treatment groups (nano curcumin gel 2% versus nano doxycycline gel 20 mg/ml ) ,the both did not show a statistically significant difference (p>0.05) in the clinical and immunological results at different follow up periods indicating that the two treatment modalities were equally effective in reducing the clinical parameters (PD, CAL, BOP) as well as the pro inflammatory cytokine IL-6.
