The effect of an early oral stimulation program on oral feeding of preterm infants in Zagazig University Hospital

Abstract

Preterm infants, especially very low birth weight and extremely low birth weight infants, are susceptible to growth failure in postnatal life if nutritional demands are not met, poor postnatal growth in preterm infants is associated with adverse neurodevelopmental outcomes during childhood.
The intervention with swallowing training made it possible to improve feeding skills in 50% of the preterm infants, starting oral feeding and obtaining exclusive oral feeding in a short period of time, with early gestational ages.
Safe and successful suckle feeding, via breast or by bottle, is one requirement for hospital discharge and an ultimate goal for preterm infant feeding and Weight Gain.
The use of an individualized, evidence-based approach is strongly recommended to assist preterm infants in transitioning from gavage to full oral feedings.
The provision forms of oral stimulation of manual peri- and intraoral stimulation followed by sucking on a pacifier that is a safe and effective strategy to enhance the feeding performance of inefficient feeders.
Pre feeding oral stimulation program enhanced the feeding performance of inefficient feeders preterm infants in terms of increased total oral intake rate, decreased net leakage and shorten the oral feeding duration observed in the intervention group when compared with the control group.
The aim of present work was to assess the performance of prefeeding oral stimulation program along with routine care over the weight gain, decrease length of hospital stay in preterm infants at 28-32 weeks of GA.
This study a prospective interventional study was conducted over an Eleven months period (from February 2017 to December 2017). The study was carried out in NICUs in Zagazig University Hospital with 58 healthy preterm infants, were studied and they were divided randomly into two group intervention group (29 preterm infants) were received pre-feeding oral stimulation program consisting of stimulation of the oral and perioral structures for 5min at least for 10 days it was started immediately after delivery and Control group (29 preterm infants) not received stimulation included 31male and 27 female and there gestational age ranged between 28-32wks. The weight of infants were measured at birth, 3 day of age, after 1week, after 2weeks, at day of introduction of oral feeding and at day of discharge.
The results of our study observe the improvement after stimulation regarding the days of life were the main days at introduction of oral feeding is 11.31 in intervention group and 15.58 in control group and at day of discharge was 16.75 and 22.31 respectively, and regard the length of hospital stay were the main is 16.76 and 22.41 in intervention group and control group and respectively.
