Abstract

”The quality and quantity of the peri-implant soft tissue affect significantly the dynamics of the marginal bone loss. So that an adequate keratinized mucosa at the implant site leads to a reduced plaque accumulation, a reduced inflammatory mucosal infiltration, and a pro-inflammatory mediator release. The need for keratinized mucosa around dental implants has been widely discussed. During the early development of endosseous dental implants, the establishment of a dense connective tissue around the implant collar for long-term implant stability was repeatedly addressed. Nowadays, in addition to achieving high implant survival following implant therapy, maintenance of functionally loaded implants in an adequate status of health and aesthetics had become a prerequisite for long-term success of implant restoration. Six enrolled patients with at least one missing tooth bilaterally which need replacement with dental implants. All the patients were selected with ≤2mm keratinized mucosa at the site of implantation . This study was a split mouth, randomized clinical trial. The patient’s randomization was done by one of the senior resident in the department, not included in the study, and not aware of any related treatment protocol. Patients were randomly distributed into: Study sides: The onlay graft of platelet rich fibrin (PRF) membrane was used around dental implant to increase the width of the keratinized mucosa in one side of each patient. Control sides: the onlay free gingival graft was used around dental implant to increase the width of the keratinized mucosa in one side of each patient. Clinical evaluation of keratinized gingiva width was performed by measuring the distance from the gingival crest to the mucogingival junction; using calibrated periodontal prob. Measurements were done before, one month and three months after second stage surgery Within the limitations of this study, it can be concluded that the onlay PRF membrane can afford increasing in keratinized mucosal width around dental implant with the advantages of a lower surgical time with less postoperative discomfort and pain for the patients in comparison to the FGG. However, FGG has significant higher ability to augment and increase keratinized mucosa around dental implants. ”
