Abstract

Infection is one of the major problems in the critical care units. Even when Antibiotics are given either for prophylaxis or treatment; the rate of infection is still high due to many factors like decreased immunity, more nursing manipulation and ie use of various invasive procedures.(2) Colonization of the lower respiratory tract by gram negative aerobic bacilli is particularly common in patients undergoing mechanical ventilation whether by racheostomy or endotracheal tube.(5) The most common organisms causing nosocomial pneumonia are the gram negative bacilli including Pseudomonas aeroginosa , Klebsiella recently maphylococcus an r ens especially methicillin resistant Staphy loco ecus aureus are [becoming important organisms for nosocomial pneumonia.(l 1) This study consists of 30 out of 50 mechanically ventilated patients who were admitted to Respiratory Intensive Care unit in Menoufyia University hospital during a period from March 2005 to September 2005 and were selected due to suspicion of developing .pneumonia according to clinical, laboratory and/or radiological evidence of chest infection. Endotracheal aspiration (EA) was done to these 30 patients on the first and fourth day of assisted mechanical ventilation in an attempt to study the significance of EA in the diagnosis of ventilator associated pneumonia, (53.4%) of cases were above 60 years old, 10 cases were non smokers while 20 cases were smokers, 13 cases had no previous history of ICU admission while 17 had previous history ICU admission and 10 cases had history of previous assisted ventilation.
Statistically .significant difference was shown in, the distribution of the organisms between Patients with early VAP vs patients with late VAP.
