Abstract

Colorectal cancer is the third most common form of cancer and the second leading cause of death among cancers in the United States. Surgery is primary treatment for most colon and rectal cancers. Some of the actual and potential problems that may be faced by patients after colostomy surgery. Assessment and management of stoma complications are often the responsibility of nurses across the continuum of care. AIM OF WORK: To identify the postoperative outcomes among cancer patients undergoing permanent colorectal diversion include; physical, social, self-esteem and nutritional changes and design proposed intervention protocolRESEARCH QUESTIONS: _ What are the different physical changes that occur in patients undergoing permanent colostomy’) What are the different social changes that occur in patients undergoing permanent colostomy? What is the effect of permanent colostomy upon self-esteem status of such group of patients? What is the effect of permanent colostomy upon nutritional status of such group of patients? SUHJECTS AND METHDS: The study sample was composed of sixty adult patients. The study was conducted in the General Surgical Units in Assiut University Hospital and Upper Egypt Cancer Institute. Data collected by utilizing the designed assessment sheet consists of demographic and medical data sheet, physical & social assessment sheet, modified nutritional assessment form, Rosenberg’s Global Self Esteem scale tRGSEs). Structured interview were utilized for data collection, which was between June2004 until June2005.RESU L TS: As regard stoma complication majority of the sample were (1.7%) had stoma necrosis postoperatively. While in follow up stoma retraction (5%), stomal stenosis (3.3%), stomal infection (8.3%), stomal fistula (5%) and mucocutaneous separation were (5%). As regard the peristornal skin condition, majority of the sample (21.7%) had skin breakdown and (21.7%) had skin irritation in follow up period. As regard the bowel evacuation changes, majority of the sample in follow up period suffered from constipation (13.3%), diarrhea (21.7%), half of the sample had gases, and (23.3%) suffered from leakage. As regards social changes majority of the sample were (58.3%) in postoperative period and increased to (6\.7%) in follow up period. The self-esteem changes were (61.7%) had low self-esteem while (38.3%) within normal self-esteem. As regards nutritional changes the present study Revealed, Males with underweight in preoperative, postoperative, before discharge and follow up periods were (50%, 64.3%, 82.1 %, 78.3%) respectively. While female with underweight were (3.1 %, 9.4%, 25%, 25%) respectively. As regard laboratory investigation subnormal value of hemoglobin in male were (57.1 %, 78.6%, 78.6%, 67.9%) respectively, while in female were (75%,.75%, 81.3%, 78.1 %) respectively. subnormal value of albumin were (36.7%, 40% 58.3%, 63.3%) respectively. while abnormal value of white blood cells were (30%, 40%, 38.3%, 25%) respectively. In conclusion, cancer patients undergoing permanent colorectal diversion exhibited different physical, social, self-esteem and nutritional changes.
