This study was undertaken to asses the practicality clarify the advantages and disadvantages of laparoscopic appendectomy and to compare it with the cul-de-sac approach for appendectomy. In our study 40 adult patients with a clinical diagnosis of acute appendicitis were randomizaly divided into two equal group;
Group I: 20 patients to whom the cul-de-sac approach for appendectomy was done.
Group II: 20 patients to whom laparoscopic appendectomy was done.
We can conclude that neither the open nor the laparoscopic technique is superior to the other. Laparoscopic appendectomy has the potential to increase the positive diagnosis especially in fertile age females and obese patients. Laparoscopy is a minimal invasive technique, with less postoperative pain, shorter hospital stay and rapid return to full activities but has two, main disadvantages which are increased , cost and long operative time. Cul-de-sac approach is used instead of the classic approaches because of its fine healing with remarkable esthetic result.
