Abstract

This study is an observational retrospective study. Our study aims at studying the epidemiological aspects, clinical, laboratory, pathological characteristics, and different lines of therapy and outcome of children with SLE at Mansoura University Children<U+2019>s Hospital. The medical records of 54 SLE patients followed from 1997 until October 2004 at lupus clinic in the Pediatric Nephrology Unit in MUCH and their data were retrieved and retrospectively analyzed. All patients were Egyptian children below the age of 16 years and were usually referred from northern part of Egypt for evaluation of renal or extra renal complaints. Mostly we stressed on laboratory data of patients and their clinicopathological correlation as well as detecting predictors of death in SLE patients. Conclusion: We conclude the following from this study: It was found that the most common extrarenal manifestation was fever (75.9%) followed by joint involvement (66.7%). Lupus nephritis was found to be prevalent in our population representing (79.6%). Proteinuria was the most evident presentation in the majority of lupus nephritis patients (81.3%) while (63%) of them had elevated serum creatinine. Among different histopathological lesions obtained by performing renal biopsies to patients with lupus nephritis, class IV was the most common pathological finding (38.1%). On performing a clinicopathological correlation between different histopathological classes with clinical and laboratory data there was significant positive correlation with clinical data rather than with laboratory data. The outcome of patients with lupus nephritis turned out to be as follows at last presentation: 54% of patients achieved remission, while (18.9%) developed disease activity, (2.7%) developed CRF. In comparison their mortality rate was (24.3%). As regards predictors of death, the most evident predictor was found to be, the severity of histopathological grade followed by nephrotic syndrome, anaemia, and C3 level.
