Abstract

This study comprised 65 patients, 55 males & 10 females with focal hepatic neoplasm (8 cases of hemangioma, one case of mesenchymal hamartoma, 35 cases of hepatocellular carcinoma, two cases of cholangiocarcinoma, two cases of emberyonal celt sarcoma, 10 cases of metastases, and 7 cases of lymphoma). All our cases examined by triphasic CT, colour & power Doppler sonography. Triphasic helical CT is an important advance over the standard CT examination in the detection and characterization of focal hepatic tumours. Hypervascular lesions are best detected during the ’hepatic arterial phase. Hypovascular lesions are best detected during the portal venous phase. The unenhanced phase is not routinely necessary for detection of liver metastases and did not help in the detection and characterization of lesions in patients with hypovascular or hypervascular malignancies. The hepatic arterial phase is especially helpful and should be routinely obtained in patients with hypervascular metastases and hepatoma. Delayed phase helps in characterization of hemangioma, as it will show progressive enhancement in this phase, while most malignant lesions will show wash out of contrast material. C.C.D.S. is a simple non-invasive method that can differentiate between benign and malignant focal hepatic lesions with high rate of accuracy, which alleviates the need for biopsy in considerable percent of patients. Although, duplex sonography alone cannot differentiate between H.C.c. and other hepatic malignancies, the colour Doppler signs are strongly positive in that aspect but not strong negative as well. It is concluded that the presence or absence of flow on CCDS is not a reliable feature for differential diagnosis .
