Abstract

Summary and conclusions This prospective study was conducted in ART unit, Tanta University ,spital and Fertility care unit Mansoura University Hospital. Forty cases were selected for this prospective study after exclusion of (male tor &immunologic infertility) ,33 cases started long protocol of COH by RHa suprefact spray & HMG ampoules and 7eases stated short protocol rwever 28 cases under long protocol and 4 cases under short protocol reach ovum ;ck-up and remained cases were cancelled due to poor ovarian response. Ovum pick-up was done for 32 cases only whom completed the trial with ,:e guide of transvaginal ultrasonography. Retrieved oocytes were assessed under ereomicroscope and inverted microscope as regard cumulus expansion and shape I cytoplasm then insemination was done . Evaluation of oocyte as regard irtllizatlon was done - 20 hours after insemination under stereomicroscope inverted microscope for detection of2PN.Thereafter the 32 cases were randomly ivided into two main groups ,co-culture group and control group. In the co-culture group the cumulus investment remains around fertilized ocyte with minimal dissection just to see the 2PN while in control group cumulus as mechanically dissected by fine needles of insulin syringes. Then embryos of oth groups were incubated in BMl medium for 24 hours. Evaluation of embryos was done 48 hours after Ovum pick-Up under &inverted microscope for detection of embryo grade to evaluate umber of blastomeres .degree of fragmentation. Embryo grades includes good ,uality (A) with regular blastomeres and no fragmentation, fair quality (B) with egular blastomeres and moderate (30%) fragmentation and poor quality (C) with rregular blastomeres and > 30%fragmentation.
