Abstract
The correlation between the effectivenes of subscleral trabeculectomy and corneal thickness proved to be a good parameter for diagnosis and prognosis of glaucoma. The aim of this work was to determine the changes in corneal thickness a~ter subscleral trabeculectomy ~or primary closed and open angle glaucoma in adultcases. Forty five eyes included in this study. They were selected from those patients admitted to Mansoura University Hospital, ophthalmology Department, for subscleral trabeculectomy. Patients were subdivided into two groups the ~irst group of primary closed angle glaucoma, were twenty five eyes with average age 55.1 ± 7.7, while the second group of primary open angle glaucoma were twenty eyes with average age 55.2 ± 8.6 or early postoperative complicur were hcluded. 1 Subscleral trabeculectomy was performed under general anaesthesia or local anaesthesia with adeqnate akinesia, fornix based conjunctival flap turned doun a 6 x 4 mm limbus-based half-thickness. Scleral flap was marked out and dissected well past the scleral spur into the cornea basal peripheral iridectomy was per~ormed through selenal trap door trabeculectomy 3 x 2 mm was done, closure of scleral flap 86 with 2-3 interrupted 8/0 virigin silk suture and Adequate suturing of conjunceiva with interrupted 8/0 virgin silk. The conneal thickness was measured by Teknar ophthasonic pachometer. The instrument composed of pachometer proper and ancillary features. The pachometer proper include the probe handle, the transduces & the probe tip. While the ancillary Fatures includ~ the numric display, s~cdor display, prob@-jack & keY’pad-Measurements were taken befor operation and on the ~irst week~ secondweeks~ first month, second months, third months & fourth months postaperatively. Both ptimary closed angle and open angle glaucoma underwent a subscleral trabeculectomy showed a increase in cOrneal thickness which was maximl on the first postope week and a gradual return to near normal thickness Within three menths.
