Abstract The relationship of the thyroid state to mania is complex. Lithium (a commonly used antimanic treatment) has rapid and profound antithyroid effect and its use may lead to frank hypothyroidism. The aim of this work is to study the effect of lithium on the hypothalamic pituitary thyroid axis in both thyrotoxic and euthyroid manic patients and also to study the relationship between mania and thyrotoxicosis as regard some clinical, psychological and biochemical parameters before and after lithium therapy. Lithium (900 mg/day) was given to 20 patients, 10 thyrotoxic (l male and 9 females) with mean age 40 years and 10 manic patients (4 males and 6 females) with mean age 32.7years.
