Abstract Critical illness is associated with a wide variety of endocrinal changes. Adrenocortical function have been studied in various forms of critical illness but the results are conflecting. The present work aims to assess the adrenocortical reserve in patients suffering from various critical illness. This study comprised 43 critically ill patients with their age 49.5+ 1.08 years compared to 15 healthy adult. The cases were divided into 4 categories: Group I: Cerebrovascular-accident (19) cases including: i- Crebral infarction (9) cases. ii- Cerebral hemorrhage (10) cases. Group II: Diabetic (12) cases including: i- Hypoglycemic (6) cases. ii- Diabetic ketosis or ketoacidosis (6) cases. Group III: Septicemia and related syndromes (6) cases. Group IV: Intoxication (6) cases. Group V: Control (15) subjects. Every cases were admitted to medical intensive care in emergency hospital of Mansoura University.
