Abstract Because the number of elderly patients undergoing surgery has markedly increased so, there will be a contiuous challenge to the anaesthetist to meet the problems associated with geriatric anaesthesia. There is close relation between increased plasma homocysteine concentration caused by intraoperative uses of nitrous oxide and development of coronary artery diseases and myocardial ischaemia the fact that proved before and reconfirmed by our study. This study was designed for searching whether the effect of sevoflurane is better than isoflurane in geriatric anaesthesia especially those suffering from myocardial ischaemia, in short time surgery like ophthalmic operations and the value of cardioprotection by these anaesthetics against the harm of nitrous oxide on the myocardium. Finally, we concluded that intraoperative uses of nitrous oxide leads to increases of plasma homocysteine which considered an independent risk factor for development of ischaemic heart diseases, even in short term operations, and the cardioprotective effect sevoflurane and isoflurane was similar. So, we recommend that there is no reason to use nitrous oxide in patients at high risk for postoperative cardiac complications. |
