Abstract

Background: Left ventricular hypertrophy was known to be one of complications of cardiovascular diseases in end stage renal disease [ESRD] patients such as atherosclerosis, cardiomyopathy, calcification, arterial stiffness, ischemic heart diseases, heart failure, cerebrovascular and cardiovascular death, Recently fibroblast growth factor 23 [FGF 23] has been identified as a cardiotoxin.
Objective: Detection of the relation between serum FGF 23 with left ventricular hypertrophy and ejection fraction in end stage renal disease Egyptian patients on regular hemodialysis.
Patients and Methods: This cross sectional study was conducted at the hemodialysis Unit, Cairo Kidney Center - Cairo- Egypt from April 2022 till December 2023. The study was conducted on (71) patients with end-stage renal disease on regular hemodialysis who were divided into two groups according to FGF23 level; group A with low FGF23 included 35 patients and group B with high FGF23 included 36 patients. All cases were subjected to full history taking, clinical examination and laboratory investigations with special emphasis on serum intact FGF 23 level using enzyme linked immunosorbent assay (ELISA). Echocardiography was also performed to assess the association between fibroblast growth factor 23 and left ventricular hypertrophy.
Results: ROC curve analysis proved poor discriminating power of the FGF23 (pg/ml) between normal LV and LV hypertrophy where area under the ROC curve at cutoff point >250.3 had sensitivity 56.8% and specificity=51.9%. There is no statistically significant difference between low and high FGF23 groups according to echocardiography regarding EF% and overall systolic function.
Conclusion: There is an association of non statistically significant value between fibroblast growth factor 23 and left ventricular hypertrophy as there was increase LV hypertrophy in high compared to low FGF23 cases but with insignificant difference. The most important factors predicting LV hypertrophy were male gender, hypertension, diastolic blood pressure and serum triglecrides.
