Abstract

Endometrial pathology is a prevalent gynecological disorder, which have a significant impact on women’s health and life. The endometrial lesions vary from simple endometrial hyperplasia to lesions with more and more complexity Which include endometrial carcinoma.
Trans vaginal ultrasound is a first step for assessment of post-menopausal bleeding showing that if the endometrial thickness is 4 mm or less has more than 99% negative predictive value for cancer. Also, histological assessment should be performed for women with frequent or persistent bleeding or cases with risk factors for EC regardless the thickness of the endometrium.
Magnetic resonance imaging (MRI) also, may be needed to evaluate the lesion’s degree and other anatomical features that may affect the decision to determine the surgery.
Computed tomography (CT) has high multiplanar spatial resolution that help in showing all metastatic lymph nodes and soft masses.
Endometrial biopsy is the main diagnostic method for variable pathologies
IETA group discussed and confirm terms, definitions and measurement techniques which are depending mainly on representive imaging of the uterine cavity.The IETA guidelines has aimed to standardize this terms, definitions and measurement techniques for ultrasound and color doppler in assessment of any pathology of the endometrium and uterine cavity.
Criteria of different pathological lesions on ultrasound according to International Endometrial Tuomor Analysis (IETA) criteria:
❖ Endometrial thickness
❖ Echogenicity [uniform, non-uniform]
❖ Endometrial midline appearance.
❖ Endometrial myometrial junction [bright edge].
❖ Intra cavity fluid.
❖ Color score.
❖ Vascular pattern.
This prospective diagnostic study aimed to establish the diagnostic performance of the International Endometrial Tumor Analysis (IETA) ultrasonographic criteria in classifying various endometrial pathologies. The study included 123 patients with endometrial pathology or symptoms suggesting abnormal endometrial patterns. Patients underwent thorough assessments, including history taking, clinical examination, and imaging applying the sonographic criteria of IETA. Following surgery, the final histopathological diagnosis served as the reference standard.
The study revealed a significant correlation between IETA scores and different endometrial pathologies, with higher scores indicating more severe pathology. Notably, the association between elevated IETA scores and the presence of malignancy underscores the diagnostic value of the IETA scoring system. Age emerged as a significant factor influencing IETA scores, aligning with previous studies indicating an increased prevalence of endometrial malignancies with advancing age.
