Abstract Most challenging problems we face as a Pedodontics is child management, how to deliver our treatment in a relaxing environment in an easiest way and avoiding any painful stimuli which exaggerates anxiety and fear. Aim of the study: To evaluate and compare the efficiency of needleless jet injection versus traditional local anesthesia injection in management of pain and anxiety during vital pulpotomy of primary molars using: 1- Visual analogue scale. 2- Venham’s anxiety and behavioral rating scale. Materials and Methods: The present in vivo study was carried out after being waived from the approval of the research ethics committee of Faculty of Dentistry Suez Canal University since it was conducted on 30 patients (a split mouth, blinded randomized controlled trial study) . The 30 patients were divided into two main age group patients: - group I (Preschool children group): 15 patients were included aged from four to six years. - group II (School children group): 15 patients were included aged from six to eight years. Then each age group was divided into 2 subgroups according to technique of local anesthesia: Traditional local anesthesia: Fifteen (15) deep carious lower second primary molars were vital pulpotomy treated using traditional nerve block local anesthesia in one side. (control side) SUMMARY AND CONCLUSION 78 Jet injector: Fifteen (15) deep carious lower second primary molars were vital pulpotomy treated using infiltration technique using needleless jet injector in the other side. (Study side). Pain and anxiety were assessed by visual analogue scale (VAS) and Venham’s anxiety and behavioral rating scale. Results: Visual analogue scale (VAS): For preschool children, results of our study showed a significant difference between the control (traditional L.A) and examined (jet injector) group immediate after LA, while nonsignificant difference between the control (traditional L.A) and examined (jet injector) group during pulpotomy. For school children, results of our study showed a highly significant difference between the control (traditional L.A) and examined (jet injector) group immediate after LA, while a significant difference between the control (traditional L.A) and examined (jet injector) group during pulpotomy. Venham’s anxiety and behavioral rating scale: For preschool children, results of our study showed a significant difference between the control (traditional L.A) and examined (jet injector) group immediate after LA and during pulpotomy. For school children, results of our study showed a significant difference between the control (traditional L.A) and examined (jet injector) group immediate after LA and during pulpotomy. All data were collected, tabulated and statistically analyzed using statistical tests. SUMMARY AND CONCLUSION 79 Conclusion It can be concluded that: Needle-less jet injector system was effective in reducing pain and anxiety during administration of local anesthesia. Infiltration by needle-less jet injector system was effective during pulpotomy procedure of lower second primary molars in preschool children, while it was effective to some extent in school age children. Traditional nerve block anesthesia was more effective during pulpotomy in school children compared with needle-less jet injector system. Fear of injection is one of the most important factors that cause fear during dental treatment in children. The child’s experience of painless and fearless dental treatment will make this person more comfortable during dental procedures in the future |
