Abstract

Background: Aplastic Anemia (AA), one of the life threatening bone marrow failure syndromes. from the main pathologies of aplastic aneamia is reduced erythropoietic activity evidenced by decreased soluble transferrin receptor (sTfR) levels results in minimal iron utilization and accumulation of iron in tissues in the form of ferritin.
Aim: To measure the serum level of (sTfR) concentration in adult aplastic anemia patients and correlate it with the severity of the disease and the response to treatment.
Patients and methods: 35 randomly selected aplastic anaemia patients recruited from haematology department aged from 17 to 66 years old and 27 healthy controls of matched age and sex. Serum of soluble transferrin receptor level (sTfR) is measured by using ELISA test.
Results: The median level of sTfR was significantly lower in cases than controls (17.9, range 9.87 – 30.42 nmol/L Vs 52.2; range 29.74 – 86.84 nmol/L; p<0.001). Serum concentration of sTfR were significantly lower in very SAA (VSAA) patients in comparison to SAA 34.27 nmol/L (31.05 – 45.41) Vs 55.15 nmol/L (48.78 – 63.88) respectively, p 0.004. Although the levels of sTfR in responders to immunosuppressive treatment was higher than non-responders {55.15 (47.36 – 65.35) Vs 48.26 (34.62 – 60.39) }, no statistically significance was achieved, P value 0.808.
Conclusion: sTfR was significantly lower in cases than controls. The sTfR concentration expresses the erythropoietic activity in aplastic patients, and is an indicator of bone marrow failure severity.
