Abstract

Acute ischemic stroke is a common and frequently occurring disease among middle aged and elderly population with severe disability and high mortality rate. In this prospective cross-sectional study, we aimed to estimate the prevalence of early neurological deterioration after successful recanalization, its potential predictors, and the patient‘s functional level after one month post stroke to assess their neurological outcomesThe study included 100 patients with AIS with a mean age of 60.64 ± 11.43 years and ranged from 25 up to 88 years old. There was male sex predominance with male to female ratio of 2.13:1. ypertension was the commonest vascular risk factor documented among our studied cases followed by diabetes mellitus, smoking, and dyslipidemia.Successful recanalization clinically or early neurological improvement (ENI), defined as NIHSS improvement by ≥ 4 with alteplase treatment at 24 hours Sixty nine percent of studied cases showed early improvement after thrombolysis and sixty eight percent still had improvement after one month.Eight percent of the studied cases were complicated by hemorrhagic transformation. The overall mortality rate among our studied cases was seven percent.Early neurological deterioration (END) is defined as increase in NIHSS score ≥ 4 points or death within 24 hours after intravenous thrombolysis, 69.0% of the studied cases showed early improvement, 27.0% have stationary course and 4.0% developed END In the current study the incidence of END after successful recanalization among our patients was 7/69 (10.1%).The explored possible independent risk factors for development of END after successful recanalization in the current study were found to be chest infection, delayed onset of treatment, higher initial NIHSS, higher initial body temperature (°C), initial and follow up higher serum glucose level, CRP, and total leucocyte count.e can say that DM itself is not the crucial factor for END after successful recanalization clinically but control of hyperglycemia is the gold standard as one on the most significant factors in our study.
