Abstract

Bladder cancer is a malignancy affecting the urothelium of the bladder, and comprises two recognized disease entities with distinct molecular features and clinical outcomes. Low-grade non-muscle-invasive tumours and muscle-invasive tumours. smoking is the most important risk factor for BC, and Painless hematuria is the most common presenting symptom of non-muscle invasive bladder cancer.
MicroRNAs (miRNA, miR) single-stranded molecules of about 22 nucleotides, are non-coding RNAs with significant roles in regulating gene expression, MiR-130a-3p can regulate cancer cell proliferation and apoptosis, MiR-130a-3p may regulate the proliferation by inhibiting PTEN expression, which activates PI3 K/AKT signal pathway (Mondal and Meeran, 2023).
Aberrant expression of miR301a-3p is associated with cancer initiation and development. It is upregulated in several types of cancer (Xu et al., 2020).
The current study was carried out at the Medical Biochemistry and Molecular Biology Department and Clinical Oncology and Nuclear Medicine Department, Faculty of Medicine, Menoufia University.
We aimed to study plasma mi-RNA-130a-3p and mi-RNA-301a-3p in patients with bladder cancer as diagnostic and prognostic markers.
It included 100 subjects divided into 2 groups: group I: It includes 50 newly diagnosed patients with bladder cancer group II: It includes 50 age and gender-matched apparently healthy individuals as a control group.

antibody and quantification of serum mi-RNA-130a-3p and mi-RNA-301a-3p using real-time PCR using SYBR Green technique.
The results of the present study can be summarized as follows:

There is no significant statistical difference between the two studied groups regarding gender and age.

88% had bilharziasis, 22% had stones, 54% were smokers, 18% had CIS 12%had DVT.18%with heart disease6%associated with diabetes,12%were hypertensive, and 2% had HCV.

40% of CB patients presented by hematuria, 58% presented by dysuria, 56% presented by frequency and 60% presented by hydronephrosis.

Significant statistically decreased hemoglobin level and a significant statistically increased urea and creatinine levels in the CB patients when compared with the controls.

66% of the cancer was transitional cell carcinoma. 60% of patients presented with high-grade cancer bladder. 92% of patients presented with stage IIIa and 72% were Ⅲb. 14% of patients presented with localized cancer. 70%of patients presented with positive urine cytology

There is a significant statistical increase between cases & control regarding the expression of mi-RNA-130a-3p and mi-RNA301a-3p.

There is a significant statistical increase in the expression of mi-RNA- 130a-3p and of mi-RNA-301a-3p in stage IIIb CB patients when compared with patients with stage IIIa.

Smoking was the most significant associated parameter with the hazard of cancer bladder (OR, 5.348 95% CI: 2.151 – 13.298), followed by mi-RNA301a-3p (OR, 5.180; 95% CI: 2.383 – 11.261), and mi-RNA130a-3p (OR, 2.797; 95% CI: 1.483 – 5.274). However, the multivariate analysis showed that mi-RNA301a-3p and mi-RNA130a-3p were the most independent factors in the occurrence of cancer bladder.

At cut off point for mi-RNA-130a-3p (>0.921) the sensitivity, specificity, Positive predictive and negative predictive values of mi-RNA-130a-3p to predict urinary bladder cancer patients are 90%, 78 %, 80.4 % and 88.6 % respectively.

At cut off point for mi-RNA-301a-3p (>1.32) the sensitivity, specificity, Positive predictive and negative predictive values of mi-RNA-130a-3to predict urinary bladder cancer patients are 92%, 96 %, 95.8 % and 92.3 %, respectively.

At cut off point for mi-RNA-130a-3p (>0.96), the sensitivity, specificity, the positive and negative predictive values for mi-RNA-130a-3p to diagnosis stage IIIa + Ⅲb are 91.3%, 75%, 97.7% and 42.9 % respectively At cutoff point for mi-RNA-301a-3p (>2.48), the sensitivity, specificity, the positive and negative predictive values for mi-RNA-301a-3p to predict stage III + IV are 76.09 %, 75%, 97.2% and 21.4 % respectively.
