Abstract

Non-surgical periodontal therapy (NSPT) is regarded the gold standard of periodontal treatment, and its clinical success is widely validated. However, new methods had been suggested as an adjunct to mechanical debridement in order to enhance its outcomes. Local drug delivery is one of the most often utilized adjunctive therapies, and tea tree oil therapy has been shown to have a positive therapeutic impact in the management of periodontitis.
The main objective of this study is to evaluate the effect of local application of 5% TTO gel as an adjunctive treatment of Stage III periodontitis regarding clinical outcomes, patient satisfaction, and the level of TNF-α.
The current study included 22 participants diagnosed with stage III and grade B periodontitis. Patients were randomized into two groups: group I, which included 11 patients treated with NSPT followed by injecting 5% Tea tree oil gel in the selected sites, and group II which also included 11 patients that acted as the control group and only received NSPT.
Clinical parameters including plaque index (PI), modified sulcular bleeding index (MSBI), probing depth (PD) and clinical attachment level (CAL) were recorded at baseline and three months postoperatively. Moreover, GCF samples were collected with perio-paper strip for biochemical assessment of TNF- α which was performed at baseline and after 3 months follow-up period. Besides, patient satisfaction questionnaires (PSQ-18 and OHIP-14) were completed by the patients three months after treatment.
Results showed no statistically significant difference between the test and control group regarding plaque index. On the other hand, there was a statistically significant reduction in MSBI and PD and gain in CAL in the test group treated by tea tree oil gel when compared to the control group.
Regarding biochemical analysis, there was a statistically significant decrease in the mean percentage change of inflammatory marker level of TNF-α in the test group.
While for the results of the Short-Form Oral Health Impact Profile (OHIP14) and the Patient Satisfaction Questionnaire Short Form (PSQ-18) revealed no statistically significant differences between the two groups.
