Abstract

The upper central incisors often break in teenagers due to trauma. Loss of tooth structure and pulp necrosis may be caused by trauma; these factors can prevent successful root completion; and the presence of a broad flared root canal and weaker dentin can make the restorative phase more difficult.
The purpose of this research was to evaluate the efficacy of the adhesive anterior endocrown vs the bespoke fiber post in the repair of a flared root canal in a human permanent upper central incisor.
Separate Invitro and Invivo sections of the investigation were undertaken.
Extracted human upper central teeth were used for the in vitro portion of the study; these teeth underwent endodontic treatment before being shortened by 2 mm at the cementoenamel junction and given a chamfered 1 mm finish. The samples were split in half, with one set getting anterior endocrowns and the other a specialized post and core.
Subgroups within each of the two groups (one of which got thermo-mechanical cycling and the other of which served as a control) were studied. After the tests were completed, each group spent 24 hours submerged in a dye solution. Dye leakage was detected by slicing the samples and viewing them under a stereo microscope.
The data was tallied and examined statistically. There was a statistically significant increase in leakage between samples subjected to thermo-mechanical cycling and control groups, regardless of the experimental methodology.
There was no statistically significant difference between the two control groups, however among the tested groups, the customized post group exhibited significantly less leakage than the anterior endocrown testing subgroup.
Twenty patients were recruited for the invitro portion of the trial and randomly assigned to either group 1 (anterior endocrown) or group 2 (customized post).
Both groups were assessed initially and again after six months using USPHS criteria. The data was collated, and then statistical analysis was performed.
When comparing restorative integrity at baseline and 6 months, neither the anterior endocrown nor the tailored post performed significantly better.
There was no statistically significant difference in tooth integrity between the two groups at baseline or after 6 months, and there was also no difference between the anterior endocrown and the individualized post.
There was no statistically significant difference in marginal integrity between the groups at baseline and 6 months, and there was no difference between the anterior endocrown and the customized post, although the latter exhibited somewhat superior outcomes.
There was no statistically significant difference in marginal discoloration between groups at baseline and 6 months, and no cases exhibited symptoms of discoloration between the anterior endocrown and the customized post.
No cases of secondary caries developed after 6 months, hence this finding cannot be considered statistically significant.
There was no significant difference in gingival health between the groups at baseline and 6 months, and there was no difference between the anterior endocrown and the individualized post. Slight hyperemia was seen in 2 of the anterior endocrown groups.
