Abstract

Chronic osteomyelitis is a debilitating condition associated with bone necrosis, soft tissue disruption and pain resulting in significant morbidity and loss of function.
The cornerstone of the treatment of chronic osteomyelitis is the radical debridement of all involved necroticand infected soft tissue and bone and well-vascularized tissue for coverage of the defect.
Soft tissue coverage after debridement can be done by various flaps that are classified into fascio-cutaneous and muscle flaps.
Our study aimed to compare the outcomes of treatment by the use of fascio-cutaneous and muscle flaps consideringfunctional outcome as flap viability and eradication of infection,aesthetic outcome, technical difficulties and donor sitemorbidity.
The study was conducted in the period from February 2021 to September 2022 for 20 patients with chronic osteomyelitis.The patients have been randomly classified into 2 groups, group (A) patientsweremanaged byfascio-cutaneous flapsand group (B)patients weremanaged by muscle flaps.
Both types of flaps were successful in defect coverage and soft tissue reconstruction but fascio-cutaneous flaps were better in aesthetic outcome and had less donor site morbidity. On the other hand muscle flaps were better in eradication of infection with less possibility in recurrence of osteomyelitis.
