Abstract

AA is the most common condition that presents with acute abdomen requiring emergency surgery. Despite its common presentation, the diagnosis of AA remains a clinical challenge. Our study aimed to compare the diagnostic accuracy of three clinical scores namely, Alvarado score, AIR score, and AAS by comparing the results of each score to the final diagnosis on histopathological basis. Our study included 100 adult patients who presented to Tanta University Emergency Hospital with a clinical diagnosis of AA and underwent appendectomy during the period from June 2021 to July 2022. For each patient, the three clinical scores were calculated, and the final histopathological result of the excised appendices proved AA so, we could only conclude the sensitivity and the false negative rate of each score and compared them to each other. We concluded that Alvarado score was the best performing score regarding overall sensitivity and false negative rate. At cut-off value >4, Alvarado score had higher sensitivity than AAS at cut-off value >10 (91% versus 80%) but without statistically significant difference. At cut-off value >4, AIR score had the lowest sensitivity (71%) which achieved statistically significant difference in relation to both Alvarado score and AAS. Alvarado score was more sensitive when used for females compared to males (91% versus 88.3%, p value <0.027), and AAS was more sensitive when used for males compared to females (86.3% versus 70%, p value = 0.041) while AIR score had equal sensitivity in both genders.
