Summary
Miscarriage is the most common complication of pregnancy, and approximately 5% of couples attempting pregnancy will experience two miscarriages, and 1% of couples will experience three or more losses. The American Society for Reproductive Medicine defines recurrent pregnancy loss (RPL) as two or more failed pregnancies, not necessarily consecutive (ASRM, 2013).
Transvaginal colour Doppler of the blood flow of the uterine arteries allows a non-invasive method for evaluating uterine perfusion. Uterine receptivity is likely to be regulated by a number of factors including uterine perfusion and is of great importance in achieving a normal pregnancy. In fact during the normal menstrual cycle, the impedance to uterine artery blood flow diminishes progressively during the luteal phase, reaching the lowest values in the period that temporally coincides with the implantation window (Selda , et al. 2012).
Many published reports show that high blood flow resistance is associated with reduced conception and that women with lower pulsatility index (PI) values have the highest possibility of becoming pregnant (Natalia , et al. 2009).
We aimed in this study to assess the uterine artery blood flow in recurrent unexplained first trimestric abortion.
 Patients and methods
A case control study was carried at El Fayoum General Hospital & Beni- Suef University Hospital starting from Sept 2019 to Feb 2020 including 60 participants divided into 2 groups:
 Case group: 30 participants presented with a history of recurrent pregnancy loss.
 Control group: 30 participants who had no history of abortion and at least 1 child born at term.
All patients subjected to pulsed wave Doppler ultrasound to obtain clear, consistent, flow velocity waveforms of both uterine arteries. Pulsatility index (PI) (PI=S-D/mean) had been measured bilaterally. The PI reported had been the arithmetic mean for the last three car cycles, in addition to measurement of endometrial thickness.
 Results
The mean of uterine artery PI is 2.69 in case group while in control group is 2.18 with significant difference between both groups (p value = 0.002). Also, there is significant difference between both groups as regard right and left uterine artery PI (p value= 0.028 and 0.015).
