Objectives: To assess the relationship between plasma B lymphocyte stimulator (BlyS) levels and disease activity in systemic lupus erythematosus (SLE). Methods: Fifty adult SLE patients, diagnosed according to the ACR revised criteria for classification of SLE, attending the Rheumatology and Rehabilitation department-Cairo university hospitals were the subject of our study. All patients were subjected to full history taking, full clinical examination, assessment of disease activity using the Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) score, routine laboratory investigations and assessment of plasma BlyS levels using Enzyme Linked Immunosorbent Assay (ELISA). Results: Plasma BlyS levels were elevated in all our SLE patients. There was a highly significant positive correlation of the total score of SLEDAI with BlyS plasma levels (P-value<0.001). There was a statistical significant difference regarding the mean BlyS values in between the patients{u2019} groups (classified according to SLEDAI score) (P-value<0.001). A significant difference of mean BlyS levels was detected between patients with mucocutaneous manifestations versus patients without mucocutaneous manifestations (P-value<0.025). The anti-ds DNA antibody titre showed a highly significant positive correlation with plasma BlyS levels (P-value<0.001). The complement levels showed significant negative correlation to plasma BlyS levels (P-value<0.038 for C3 and P-value<0.007 for C4). No significant relations were detected between other variants{u2019} used in calculating the total SLEDAI score and the plasma BlyS levels
