Modern life has been characterized by many amenities and luxury as a result of the tremendous and continuous scientific progress, which has led to the exposure of modern man to repeated stress, especially for the spine.
But sometimes, through the nature of his work, a person is exposed to some health and movement problems that hinder him from fulfilling the requirements of his job or practicing his life normally.
Samia Khalil (2008) mentions that back injuries are rare, but they may cause spinal cord injury, which is a great danger. Back injury may cause loss of feeling, numbness, weakness in the lower extremities, and some acute back injuries cause neurological damage, so they need special medical care and occur as a result of low severity Frequent such as excessive bending forward and backward, as well as anatomical distortions, wrong postures, natural weaknesses in the muscles, and cases of atrophy associated with age, as well as inactive people may suffer from lower back pain and athletes due to stress.
Ayman Al-Husseini (2009) points out that the moving vertebrae of the spine include four natural curvatures, two of which are convex back, which are the dorsal curvature and the sacral curvature, and two of which are convex forward, the lumbar curve and the cervical curve. .
In this regard, Sarah Burkart (2001 AD) indicates that back pain is one of the reasons why a large proportion of people (80: 90%) visit doctors’ clinics, where they are exposed to injury through daily hardships of work or exercise, and usually Rolling and flexing movements form a large amount of pressure on the vertebrae, which increases over time, and the pain is mechanically due to injury to vertebrae, cartilage, ligaments, or muscles of the lower back. To 50% in the age between 30 to 50 years, and most of the people gradually improve through exercise, muscle stretching and anti-inflammatories, not surgery.
Both Muhammad Qadri Bakri and Siham Al-Sayed Al-Ghamry (2011) point out that the use of various types of physical therapy, whether through physical exercises, playing, swimming, or otherwise, all work to strengthen and improve the patient’s condition in general, as well as increase awareness and awareness of various matters in particular. What surrounds it of nature and human and spatial fields and helps the effects resulting from physical therapy.
Problem of The Research:-
from the foregoing, and through the researcher’s work as a sports specialist at Beni Suef University, he noticed that there are complaints from a large number of employees at Beni Suef University, clerical staff, about being exposed to lower back pain and an attempt by the researcher to solve the problem of this important group of university workers, which may have a negative impact on me. The productive aspect of the university, and it may also be a reason for the beginning of serious injuries to workers, which may affect their health and psychological state, which may hinder their practice of their normal life, which is reflected on their ability to communicate and perform and their inability to continue to work, which led the researcher to address this problem by proposing a rehabilitation program Physical therapy and massage and studying its effect on lower back pain for this group of university employees.
Goals of The Research :-
The research aims to:
Designing a physical rehabilitation program and using massage to reduce lower back pain and some basic functions of the spine, according to the requirements of the professional work of Beni Suef University employees, and studying its effect on:
1- Pain level
2- Abdominal and back muscle strength
3- Kinetic range of the spine (left - right - front - back)
Hypotheses of The Research:-
1- There are statistically significant differences between the mean of the tribal and posterior measurements of the research sample in the level of lower back pain in favor of the post-measurement.
2- There are statistically significant differences between the mean of the tribal and remote measurements of the research sample in the level of strength of the abdominal and back muscles in favor of the post measurement.
3- There are statistically significant differences between the mean of the tribal and posterior measurements of the research sample at the level of the motor range of the spine and in favor of the post-measurement.
Methods of The Research :-
The researcher used the experimental method by following the pre and post measurements for one experimental group due to its relevance to the nature of the research.
Community of The Research :
The employees of Beni Suef University are clerical employees and those with lower back pain, and their number is (40) employees.
Sample of The Research:
The researcher chose the sample in a deliberate way from the employees of Beni Suef University and those with lower back pain, their age ranged between (50-55) years, and their number was (23) employees, including (4) for the exploratory sample and (19) for the experimental sample.
The terms of selection of the sample:
1. Desire to implement the rehabilitation program and submit a written evidence of that.
2. Not to be exposed to any other spinal injuries that affect the implementation of the rehabilitation program
3. He should not be subject to any other rehabilitation exercise programs.
4. Regular application of the qualifying program units throughout the program period
Means and tools of data collection:-
a. Data registration form: - It means a form for recording data for each case separately, attached () and includes data (age - height - weight - flexibility of the spine - strength of back muscles - strength of abdominal muscles - degree of pain)
B. Devices used:-
1- The restameter
2- The dynamometer
3- Medical scale
4- The length of the tape measure is included in centimeters
5- Visual analogue scale (V.A.S) to measure the degree of pain
6- stopwatch
Second, Physical Tests:
Abdominal exercise (to measure the strength of the abdominal muscles).
General framework for the implementation of the program:-
To design the proposed program, the researcher conducted a reference survey of the studies related to the subject of the study, and the results were as follows: The duration of the program should be three consecutive months, with a total of 24 training units and 12 massage sessions, with two training units per week and one massage session per week, provided that the time ranges between The unit is from (25-45).
exploratory studies:
In light of the research problem and the method used, the researcher conducted an exploratory study before implementing the proposed program in the period from 14/11/2021 to 16/11/2021 on a sample of (4) individuals from the research community and outside the main sample with the aim of:
1. Determine the number of assistants and train them to assist the researcher in conducting the study.
2. Determining the suitability and validity of the tools and devices used in the program and the measurements.
3. Ensuring the validity of the program implementation places, identifying problems and difficulties that may encounter the program or measurements, and finding appropriate solutions to them.
Survey results:
1- Determining the number of repetitions and the performance time for each of the suggested exercises and resting between each exercise and another.
2- Determining the suitability and validity of the tools and devices used in the program and measurements.
4- Knowing the use and operation of the dynamometer and testing its validity and stability.
Executing search measurements:
Tribal measurement:
The researcher conducted a tribal measurement of the research sample in the Sports Activities Hall of the General Administration of Youth Welfare at the University of Beni Suef on Wednesday and Thursday corresponding to 1 . 2/12/2021 AD. weather, time of day).
Implementation of the qualification program:
The researcher applied the physical program to the experimental group during the time period from Sunday 5/12/2021 AD to Sunday 22/2/2021.
Dimensional measurement:
The researcher conducted a post-measurement in the variables under study, after the completion of the implementation of the qualifying program on Tuesday and Wednesday 1,2/3/2022 AD.
Statistical processing:
The researcher used the following statistical methods for suitability to the nature of the research , namely:
It has embraced the researcher significance level ( 0.05) , and has hired a researcher statistical program (SPSS)
Conclusions:-
In light of the search results within the limits of research and procedures plan could be reached the following conclusions:
(1) The implemented rehabilitation program led to an improvement in the level of pain reduction, and this appeared through an increase in the improvement in the rates of change of the follow-up and dimensional measurements from the pre-measurements.
(2) The implemented qualifying program led to the development of the flexibility of the trunk and the range of motion of the trunk and the lumbar region in all directions and an increase in the speed of performance. This was evident through the increase in the improvement of the rates of change of the sequential and dimensional measurements from the tribal measurements.
(3) The implemented rehabilitation program led to the development of the muscular strength of the back muscles, and this was evident by increasing the improvement in the rates of change of the follow-up measurements and the dimensional measurements from the tribal measurements.
(4) The implemented rehabilitation program led to the development of the muscular strength of the abdominal muscles, and this appeared through the increase in the improvement of the rates of change of the sequential measurements and the dimensional measurements from the tribal measurements.
Recommendations:-
Depending on the data and information that enable the researcher reached Guided ” In the framework of the conclusions of the research sample limits the researcher recommends the following:
(1) Be guided by the proposed rehabilitation program for men with lower back pain from (50-55) years old.
(2) Applying the program to a larger number of government agencies with office work, in order to improve their physical and motor performance.
(3) The need to pay attention to continuing the rehabilitation exercises for developing the muscle strength of the trunk muscles, and the flexibility exercises of the trunk that maintain the range of motion of the lumbar region in all directions after the completion of the rehabilitation period, in order to avoid the aggravation of the injury to advanced degrees.
(4) Attention to design rehabilitation programs using natural means to prevent exposure to lower back pain for different groups of both sexes and according to the type of work practiced.
(5) Implementation of the initiative of the President of the Republic, an hour of exercise daily for all employees of government departments, to improve the general health status of all.
