Abstract
Background: Ultrasound is a minimally invasive diagnostic technique that allows real-time sampling of mediastinal and hilar lymph nodes
Aim of the work: to evaluate the role of different modalities of Ultrasound guided procedures in diagnosis of mediastinal lymphadenopathy.
Patients and methods: The study was performed on 79 patients who had been referred to the Chest department, Kasr El-Aini Hospital for clinical assessment, out of these 79 patients, 24 patients underwent cervical LN biopsy, 23 of them reached final diagnosis and one patient did not reach final diagnosis and needed excisional LN biopsy.31 patients underwent TUS guided mediastinal LN biopsy and all of them reached final diagnosis. 24 patients were subjected to CP-EBUS guided biopsy, 20 patients reached final diagnosis, 4 patients did not reach specific diagnosis, 3 of them went for mediastinoscopy and 1 patient had scapular deposit from which US guided biopsy was taken.
Results: Ultrasound has sensitivity 93.67% and positive predictive value 100% in diagnosis of mediastinal lymphadenopathy.
Conclusion: Ultrasound is minimally invasive procedure for diagnosis of mediastinal lymphadenopathy with high diagnostic accuracy and every patient with mediastinal lymphadenopathy should be subjected to US screening for accessible cervical and mediastinal LN.
