Abstract

Objective: Evaluation of the efficacy of low dose dexamethasone on ovarian response and pregnancy outcome in infertile type A Polycystic ovarian syndrome.
Methods: The study included the infertile patients with type A polycystic ovary syndrome which were candidate for In Vitro Fertilisation/ Intracytoplasmic Sperm Injection(IVF/ICSI) using fixed GnRH antagonist protocol and randomized into two groups; low dose steroid group that included 80 women with type A PCOs undergoing a trial of IVF/ICSI and received two tablets of dexasone 0.5 mg each night from the first day of gonadotrophin administration till the night before oocyte retrieval with GnRH antagonist controlled ovarian hyper stimulation (COH) and placebo groupthat included 80 women with type A PCOs undergoing IVF/ICSI who received two tablets of placebo commencing on the first day of gonadotrophin administration until the night of oocyte retrieval.
Results: In low dose dexamethasone group, stimulation duration was statistically significant shorter, stimulation ampoules were significantly fewer however, E2 on day of trigger, fertilization rate, number of retrieved M2 and fertilized oocytes were significantly higher compared with placebo group. On the other hand, there was no significant difference between both groups regarding incidence of OHSS and rate of embryo transfer.
Conclusions: Dexamethasone enhances quality of embryos and pregnancy rate; in addition, it reduces gonadotropines ampoule used for stimulation, hence, we recommend using of dexamethasone in women with PCOs under treatment of IVF/ICSI.
