Abstract

The ideal biomarker for atherosclerosis should identify both the degree of vascular injury and the regenerative capacity of the body. The EPCs may be used as a measure of vascular homeostasis. In this study we aimed to assess the value of EPCs as a surrogate marker of the infarction size and left ventricular function after AMI. The number of the peripheral blood CD34 + /133 + were determined in 33 patients at day 1 and day 7 after AMI. All patients had infarction segments assessed using cardiac ceMRI scan within day 7 after AMI. All patients had follow up echocardiography at 6 months to assess left ventricular remodelling. Results revealed Negative correlation with statistical significance was found between number of CVFRs and number of mobilized EPCs after AMI (P < 0.001; r² = - 0.33). Similarly the relationship between the infarction size and number of infarction segments were negatively correlated with numbers of mobilized EPCs after AMI (P < 0.001; r = - 0.82), (P < 0.001; r = - 0.86) respectively. During period of 6- months follow-up better echocardiographic indices (EF and LVESV) were shown in those who were able to mobilize more EPCs. In conclusion spontaneous mobilization of EPCs can serve as an important marker for infarct size. In addition early sSpontaneous mobilization of EPCs can predict improvement of LV function and favorable cardiac remodelling
