Abstract

Background: To evaluate value of vaginal fluid Ý-hCG in the diagnosis of rupture of membrane among patient with history suspicious of amniotic fluid leak. methods:A total of 90 pregnant women with Gestational age {u2265} 32 weeks participated in this study divided into 3 groups.group I (intact membrane), group II (confirmed ROM) &&group III (Suspected but unconfirmed ROM). All patients underwent a sterile speculum examination for detection of amniotic fluid pooling & Nitrazine paper testing followed by vaginal fluid sampling (4 cm). BhCG detection was doneusing hCG dipstick & ELISA. Suspected group patientswere followed up for 2 weeks by clinical examination and ultrasound examination(to confirm or disconfirm the diagnosis of ROM).results: The mean vaginal fluid hCG levels in group (I), group (II) and group (III) were 2.5 ± 3.9, 355.9 ± 24 and 228.817 ± 43.89 mIU/mL respectively, The difference was statistically significant (P= 0.000). WithhCGcut-off value of 180.8 mIU/ml, the sensitivity, specificity, positive and negative predictive valuesin confirming ROM are 93.5, 75,79.6 & 91.7% respectively. conclusion: Ý-hCG titer was significantly higher in cases with definite ROM.Consequencely, vaginal fluid hCG can be used asan easy, rapid, reliable and non-invasive test for confirmingthe diagnosis of ROM and can be used as a adjunctive test in equivocal cases
