Abstract

This prospective randomized study was performed to evaluate the role of ejaculatory preserving transurethral prostatectomy technique in preserving ejaculation.
1st goal to evaluate ejaculatory preservation function advantage 2nd goal to monitor voiding outcome of TURP by measuring IPPS ,Q max, sexual questionnaire pre and post-operative.
All patients were evaluated pre operatively by detailed history and physical examination, routine laboratory investigations and imaging including abdominopelvic ultrasonography, Uroflowmetry
IPPS ,Q max, sexual questionnaire were measured preoperatively and 4 weeks and 3 months post-operative. Also intraoperative and postoperative clinical data were collected tabulated and analyzed including operative time, intraoperative blood loss, post-operative hospital stay. The patients were divided into two main categories ejaculatory preserving transurethral resection of prostate group and ordinary transurethral resection of prostate.
There was no difference between groups in preoperative laboratory parameters. We found that symptomatic improvement was noticed as mean (IPSS) at 4 weeks postoperatively with insignificant difference between the two groups. Regarding Q max there was no difference between the two groups in the increase that happened in post-operative by.sexual quality of life (S QoL) postoperatively was significant increase in both groups with preference improvement index towards epTURP.
Summary and Conclusions

Out of 30 cases underwent epTURP 24 cases reported the ability to have antegrade ejaculation, but 6 cases didn’t, for standard TURP 30 cases group, 14 cases were able to have antegrade ejaculation ,but 16 cases did not.
Conclusions
We found that ejaculatory preserving TURP technique is superior to classic TURP technique in preserving ejaculation.
Ep TURP technique is comparable to classic TURP in post operative Voiding Function Parameters.
