Abstract

Objective: The objective of this study is to evaluate the efficacy and safety of intra venous paracetamol versus oral Ibuprofen in the treatment of hemodynamically significant patent ductusarteriosus (hsPDA) in preterm infant Study design:our randomized study was conducted on 60 preterm infants with gestational age < 37 weeks, Doppler echocardiographic evidence of hsPDA. Neonates were randomly assigned to two groups: group 1 (paracetamol group) 30 neonates received 15 mg /kg/6h IV Paracetamol infusion for 5 days. group 2(Ibuprofen group) 30 neonates received 10 mg/kg oral Ibuprofen followed by 5mg/kg/day for 2 days. With failure of ductal closure, a second course of ibuprofen or paracetamol was given of the same duration.The included newborns were subjected to detailed history, clinical examination, laboratory investigations that included complete blood count, renal and liver function tests and echocardiographic evaluation. Results: IV paracetamol was as effective as ibuprofen for the closure of patent ductusarteriosus (PDA) with one course of treatment (p{u2009}>{u2009}.05).There was significant increase creatinine (p=0.044),Risk of GIT bleeding(p=0.001) and reduction in platelet count (p=0.03) in Ibuprofen group. Research also showed elevation of AST level in Paracetamolgroup (p=0.028)
