Abstract

Since dental implant is best option for replacing teeth when feasible, flapless immediate implant is studied in this thesis to assess the use of BCP material as filling material for jumping gap as it was recommended to be used as an alternative for other bone graft materials.
The aim of the study was see effect of BCP on labial plate of bone over 6 month of healing and to compare results with xenograft as control clinically through gingival biotype and attached gingival width. Second aim was to assess the peri-implant soft tissue through an esthetic score the PES.
Patient were selected from Ain Shams clinics, with hopeless remaining anterior or premolar tooth indicated for immediate implant, aged between 20 and 50 years old , medically free, not smoker with think gingival biotype, has at least 2 mm of keratinized tissue and adequate oral hygiene. 20 patients was selected and grouped randomly into one of the two groups through flipping the coin.
Three CBCT was taken, prior the surgery for assessment, at day of surgery, and after 6 month of healing to be used to record vertical and horizontal labial plate dimensions at the central part.
Preoperative clinical parameters were recorded; attached gingival width, gingival biotype, and PES and later after 6 month of healing.
Atraumatic extractions is performed with help of periotome instrument, followed by drilling and placement of implants (F.M.D implant) with the appropriate length and diameter leaving between 2-3 mm gaps from labial plates , then augment the gaps with bone and cover by collagen cone and suture over to hold the cone in place. Postoperative instruction and medication is performed, and suture removed after 10 days.
CBCT at the day of surgery and one after 6 month were superimposed to measure the vertical and horizontal labial plate dimension at three levels.
The study revealed that BCP have similar result as xenograft in the gab distance with relatively similar bone loss percentage of labial plate vertically and horizontally and acceptable PES score similar to control group. While the attached gingiva width and gingival biotype changes were insignificance between two groups.
Study group has less percentage of bone loss vertically yet the difference was not statistically significant (p=0.1530). also study group showed less percentage of bone loss in horizontal dimension at 2mm level yet the difference was not statistically significant (p=0.901).
Study group showed higher percentage of bone loss than control group than control group loss in horizontal dimension at 4 and 6mm level yet the difference was not statistically significant (p=0.705), (p=0.200) respectively.
