Abstract

Summary
The aim of the present study was to evaluate the effect of a developing rehabilitation exercises program on lower limb joints function for burned patients.
The following tools were utilized for data collection:
Tool I: Patient’s assessment: this tool consisted of two parts.
Part A: Demographic patient data:
It was developed to assess demographic characteristics. It included age, sex, marital status, level of education and occupation.
Part B: Patient medical data:
It was developed to assess past and present medical history. It included cause of burn, degree of burn, percent of burn, first aid, skin graft, joint affected, date of admission and date of discharge.
Tool (II): Severity Rating Contracture Scale (pre& post): By using severity rating contracture scale which was developed by (Schneider et al., 2006).
This scale divides contracture into three levels (mild, moderate and severe contracture).
This tool was applied by the researcher by using a goniometer with a standardized technique (Norkin and White,, 1958).
This part was assessed four times (pre-program when the patient admitted in the burn unit, immediately post program, 2 weeks and 6 weeks post program.
Scoring system:
According to the severity rating contracture scale, the degree of contracture will be specified into three levels, mild, moderate and severe contracture (As the following table).
Data were collected & analyzed by computer-programmed spss (ver. 6). Using necessary statistical manipulations as; number, percentage, mean & chi- square test to determine significance for variables. T. test used to determine significance for numerical variables. Statistical significance was considered at p- value <0.1
Main findings of the present study were as the following:
The mean age of patients were (32.5 ± 8.7) years old. The highest percentage of studied patients were males, married and have university education (73.3%, 63.3% ,63,3% respectively). Regarding occupation, it was found that the highest percentage was non employer (36.7%).
The majority (96.7%) of the studied patients had no chronic diseases, the main cause of their burn was flam followed by scald (66.7%, 33.3% respectively) and all (100%) the studded patients were represented by mixed degree burn (1st and 2nd ). The mean hospital stay of the studied patients was (21.5 ± 8.39 days).
The highest percentage of studied patients had knee burn (56.7%) of the right leg , and hip burn (56.7%) of the left leg .
There was a statistical significant difference as regarding lower limb joints function between pre, immediately and 2 &6 weeks after implementation of rehabilitation exercise program. There was an improvement in the degree of contracture of burned lower limb after implementation of rehabilitation exercise program. In which the degree of contracture was changed from severe and moderate to mild in different lower limb (hip, knee, ankle).
That a statistical significant difference was found between assessment of lower limb joint function obtained pre & post implementation of exercise program. P. value (< 0.05).
There was a statistically significant correlation between TBSA and knee flexion as well as ankle extension (0.0277 and 0.035 respectively) immediately post program. Also there was a statistically significant correlation between TBSA and knee flexion and ankle flexion 2 weeks post program (0.024 and 0.042 respectively). Moreover, there was a statistically significance correlation between TBSA and hip, knee and ankle flexion 6 weeks after implementation of rehabilitation exercise program (0.043, 0.014 and 0.044 respectively).
