Abstract

Thyroid nodule is defined as a focal well-defined area of altered echogenicity within thyroid gland that is radiologically distinct from surrounding normal thyroid parenchyma. Thyroid nodules prevalence by palpation alone is 2-6 % while by ultrasound examination it reaches 19-35%. In the recent decades, a remarkable increase in the incidence of thyroid carcinoma has been noted due to markedly improved ultrasonography surveillance and sonar-guided FNAC of thyroid nodules The aim of this study was to assess the role of the TIRADS scoring system in discriminating malignant thyroid nodules from benign nodules and to investigate the association between thyroid nodules and metabolic parameters.The present study included 111 Egyptian patients (106 females and 5 males with a mean age of 40 ± 12 SD) with euthyroid nodules, referred to endocrinology clinic at Cairo university hospital in the period from January 2020 to December 2020. All of them underwent thyroid ultrasound. Categorization of each nodule according to the TIRADS ranged from 1 to 5. Final diagnosis was done by Bethesda system cytology classification. There were 19 malignant nodules and 92 benign nodules.There were statistically significant differences between benign and malignant nodules regarding TIRADS classification, taller-than-wide shape, solidity,border,presence of peripheral calcifications or punctuate echogenic foci ( p<0.05). Taller-than-wide shape had the highest specificity followed by irregular margin (94.6% & 92.6%).Sensitivity, specificity, PPV, NPV for ACR-TIRADS versus histopathology were respectively 73.7%, 57.6%, 26.4% and 91.4 % with overall accuracy 60.4%
