Abstract

Background:Medication error and adverse drug reactions occur frequently, leading to a high burden of patient harm in the hospital setting.The errors may reach 50% during prescribing, 9% during pharmacy dispensing and 40% during administration phase. Aim: This study aimed to describe how work was organized at Theodor Bilharz research institute and identify critical points, pros and cons, in the medication process and its repercussions on the job demands of the healthcare teams. Methods: This was an exploratory, descriptive and qualitative study conducted at Theodor Bilharz Research Institute Hospital, GIT department. The data collection took two months. The sample included all the healthcare workers having a role in the medication process; 14 junior physicians,15 nurses and 4 pharmacists. The participants answered the study questionnaire, were observed during their daily work and some of them shared in reviewing the medical records for two consecutive months. Results: Most of the healthcare team members evaluated the medication safety measures in the hospital as fair to good. Physicians reported heavy workload and interruptions as the most common probable risks on medication safety; 64.3% for each followed by stress, lack of guidance and unavailable lab results; 57.1% for each. Participant pharmacists reported short staff, illegible physician{u2019}s handwriting; 100% and 75% respectively. Participant nurses reported absence of medication safety guidelines, insufficient experience/ training, working in a rush, doing multiple tasks at the same time and exhaustion; 66.7% for each followed by absence of rechecking of the ordered medications; 53.3%
