Abstract

Background: Nutritional support as enteral or parenteral nutrition is used in hospitalized patients to reduce catabolism.Enteral nutrition (EN) is recommended as the preferred route for early nutrition therapy in critically ill patients over parenteral nutrition (PN). Objective: This interventional clinical trial aims to compare the outcomes of early EN with early PN in critically ill children with respiratory distress. Methods: study was conducted on 180 patients, which randomly subdivided to (90) children received EN and (90) children received PN.All patients were subjected to full history, physical examination, nutritional assessment, and labs, also pulmonary indices were calculated andoutcome were recorded. Results: The higher rate of infection and mortality was founded in the group who received PN when compared with EN group.There was highly statistically significant difference as regards sepsis between EN group(16.7%) and PN group (41.1%) with P value=0.001.The mortality rate in PN group was (27.8%) which is higher than EN group (11.1%) with P. value=0.01, median of weight gain on the EN group was 0.17 kg at second week which is more than those in PN group with statistically significant difference between both groups with a P.value =0.001 and there was no statistical significant difference as regard PICU stay length and mechanical ventilator stay length
