Abstract

In pregnant woman, Lipid profile changes during pregnancy are a result of physiological adaptation to the state of pregnancy. There are increases in the blood concentration of Cholesterol, Triglycerides, LDL cholesterol and decreases in HDL cholesterol. Blood lipid concentration in the plasma increase significantly during pregnancy in response to estrogen stimulation and insulin resistance. The lipid levels are also affected by rise in progesterone and human placental lactogen. Other maternal factors such as BMI (body mass index), maternal weight gain, maternal nutrition, pre-pregnancy lipid levels, and various medical complications of pregnancy (e.g., DM) may also have significant effects on lipid metabolism and plasma levels. The aim of this study was to evaluate lipid profile in second and third trimester of pregnancy and to find out its correlation to maternal and fetal outcome. This prospective cohort clinical study held in antenatal care unit in outpatient clinic of obstetrics and gynecology in Mansoura university during the period between June 2018 to June 2021, 189 normal pregnant women were enrolled in this study with mean age 24.78±4.74 years, with mean BMI 27.58±4.16. All of these women were subjected to history taking, general and abdominal examination and measure lipid profile in second and third trimester. In this study we found that the mean values of serum cholesterol level, serum triglycerides, serum HDL and serum LDL during second trimester were 210.14±27.22 mg/dl, 160.85±18.55 mg/dl, 55.32±10.25 mg/dl, 120.01±29.39 mg/dl respectively but, during the third trimester were 239.95±27.39 mg/dl, 191.65±18.92mg/dl, 45.62±10.25 mg/dl, 135.36±21.49 mg/dl respectively, There is a statistically significant difference in serum cholesterol, serum triglycerides, serum HDL and serum LDL levels in third trimester of pregnancy (P values <0.001, <0.001, <0.001 and <0.001 respectively) compared to second trimester of pregnancy. Regarding maternal and fetal complications we found that, 7 cases (3.7 %) had Preeclampsia, 9 cases (4.8 %)had PIH, 10 cases (5.3 %) had preterm labor, 8 cases (4.2 %)had GDM, 17 case (9.0 %) had SGA, 17 case (9.0 %) had LGA and 3 cases (1.6 %) had still birth. It seems that there are relations between maternal and fetal outcome and lipid profile in second and third trimester.
