Abstract ABSTRACT Background: Podocytes are unique cells with a complex cellular organization with respect to their cytoarchitecture, podocytes may be divided into three structurally and functionally different segment: cell body, major processes and foot processes. The foot processes of neighboring podocytes regularly interdigitate, leaving between them the filtration slits that are bridged by an extracellular structure, known as the slit diaphragm. Aim and Objectives: To asses the value of urinary podocalyxin as an early marker for podocyte injury in patient with diabetic nephropathy as well as other glomerular disease and its use in comparison between diabetic nephrophathy and glomerular disease. Subjects and Methods: A case control study was conducted at the internal medicine and clinical pathology departments of Ain-shams University Hospitals from April 2016 to July 2017 on 80 patients divided into: 40 patients with glomerular diseases but not diabetic, 40 patients with Diabetic nephropathy divided into: 20 patients with macroalbuminuria and 20 patients with microalbuminuria and 20 apparently healthy subjects serving as control. Result: This study comprised 80 patients(34 females& 46 males) subdivided into 2 groups: (GI) 40 patients with glomerular disease (16 females & 24 males), (GII) 20 type 2 diabetic patients with macroalbuminuria (11 females & 9 males), (GIII) 20 type 2 diabetic patients with microalbuminuria (7 females & 13 males) and 20 apparently healthy subjects subjects served as a control group (GIV) (4 females & 16 males). Conclusion: F rom the results of this study and due to the characters of podocytes, which are located on the outline of glomerular filter barrier and shed into urine after injuries to podocytes, only detecting urine podocytes can evaluate glomerular filter lesion irrelevant to the other two components (endothelial cells and GBM), suggesting that podocytes injury occurred with the appearance of microalbuminuria. PCX being a special protein of podocytes, so its detection in urine indicates Pods injury. |
