Abstract

This study aimed to asses corneal densitometry and ABCD grading progression before and after corneal collagen cross-linking for keratoconus using pentacam. This was an experimental interventional prospective study included 73 eyes with keratoconus indicated for corneal collagen cross-linking. recruited from Mansoura ophthalmic center, Mansoura University, Egypt, within the period between November 2019 to November 2020. Corneal topographic parameter ( Kmax,corneal astigmatism, cental corneal thickness, anterior ,back elevation, ARTmax and keratoconus grading) were measured using automatically rotating Scheimpflug camera (Pentacam). corneal densitometry parameters and the three parameters [corneal thickness at the thinnest point and anterior and posterior radius of curvature (ARC, PRC) taken from the 3.0 mm optical zone centered on the thinnest point] were measured by using automatically rotating Scheimpflug camera (Pentacam). The results of this study showed that :  There was a statistically significant increasing in CD between pre-treatment and one month post treatment. then from the 3rd month to the 6th month, clarity improvement is witnessed and CXL-correlated corneal haze appears to decrease significantly.  There was a statistically insignificant corneal thinning. These initial insignificant changes we think may be attributed to the corneal de-epithelialization that was performed during the operative procedure.  There was a stability with significant reduction of the values of anterior elevation and posterior elevation after 6 months after CXL.  There was a statistically significant improvement in BCVA at 6 months post-treatment.  There was a stability of the corneal astigmatism with non significant reduction was noticed.  There was a statistically significant improvement in Kmax after cross linking.  There was no statistically significant change in ARC or PRC when comparing to the preoperative stage.  There was no statistically significant increasing in ARTmax at 3 months which gradually return to baseline at 6 months.
