Abstract

background: CHD was demonstrated to be the second most prevalent chronic illness in childhood period. In terms of clinical level, it could be classified as cyanotic or a cyanotic according to the percentage of oxygen saturation in the blood. CHD includes a broad spectrum of simple, moderate and complex heart lesions and it is the commonest birth defect. In the past, life expectancy of children born with cardiac disease has increased considerably. They could make it to adulthood due to advances in diagnostic and medical management and improvements in cardiac surgical approaches. Cardiac diseases were demonstrated to be associated with abnormalities to the development of children and adolescents and thus trigger emotional and behavioral (internalizing, externalizing) troubles. Aim of the work : This study aimed to determine the effect of CHD and its therapeutic intervention on the behavior and emotion of the children with CHD and allow early intervention to prevent persistence of these troubles into adolescence and adulthood. Materials and Methods : This study was an observational case control study held on pediatric cardiac department and outpatient’s clinic of Mansoura University Hospital within 6 months on (60) children with CHD and (60) control whom age was between 6-18 years. Results : Significance increases in both internalizing and total score in CHD cases compared with healthy controls and in severe cases of CHD compared with mild and moderate cases. Clinical Total and Internalizing CBCL category were more prevalent among those receiving medical treatment than those not under medical therapy. Internalizing CBCL Score showed significance increase in moderate disease cases in comparison with mild cases and in CHD cases compared with controls. Total CBCL Score showed significant increase in cases with severe disease in comparison with mild cases and in CHD cases compared with controls. Mean Internalizing score was significantly higher among Cyanotic than A cyanotic cases and Controls while the mean Total score was significantly higher among A cyanotic than Controls. most of those with Cyanotic group had clinical Internalizing CBCL. Clinical Total CBCL category was more prevalent among low socio-economic status than moderate socio-economic status. Subclinical Externalizing CBCL category was more prevalent among 12-18 years old than among 6-12 years old. Moreover, most of those who are 6-12 years old (95.6%) have normal Externalizing CBCL. Conclusion : children with CHD have behavioral and emotional troubles related to their cardiac disease. There is high prevalence of emotional and behavioral troubles in children with CHD, Internalizing and total troubles are more prevalent in children with CHD especially those with sever CHD and/or receiving medical treatment than healthy control and mild, moderate cases. Also, children with cyanotic CHD exhibiting more internalizing and total troubles than a cyanotic and control one. Subclinical externalizing CBCL was more prevalent among 12-18 years old than among 6-12 years old. Moreover, most of those who are 6-12 y ears old have normal CBCL. Finally, Clinical total CBCL was more prevalent among low socio-economic status than moderate socio-economic status.
