Abstract

The nose inhabits the central position of the face and is often the most dominant part of the facial profile, and nasal profile aesthetics may have profound emotional and sociocultural significance for patients.Rhinoplasty surgeon always looking for best techniques and to decrease post-surgical morbidities (oedema, ecchymosis, and pain).The first application of PEI in rhinoplasty surgery was reported by Robiony in 2004 and published in 2007. The initial publication reported on the use of a piezo scalpel for performing lateral osteotomies through a percutaneous approach.The current study was implemented to evaluate and compare the clinical outcomes between Piezosurgery technique against a conventional osteotomy technique for lateral osteotomy and rasping in open-technique rhinoplasty. Our study included 40 patients (20 done with piezo, 20 traditional osteotomy). Our results demonstrated that Piezosurgery reduces edema, pain, and ecchymosis after surgery.In our work we found that pain was less and significant with piezosurgery at 2nd day postoperative, edema was less and significant at 2nd and 4th day postoperative with piezosurgery and insignificant at 7th day postoperative between both groups, ecchymosis was less and significant at 2nd and 4th day postoperative with piezosurgery and insignificant at 7th day postoperative between both groups, duration of surgery was shorter in conventional technique than piezo but insignificant. So, we recommend piezosurgery technique than conventional technique in lateral osteotomy and hump removal in open rhinoplasty.Advantages of piezosurgery include the selective cutting of mineralized structures, which avoids soft-tissue injury and achieves more precise greenstick fractures, preservation of soft tissue, and maintenance of nasal function.Disadvantages of piezosurgery compared with conventional osteotomy techniques that piezo surgery is coasty, duration of surgery is longer than traditional technique (specially at the beginning of piezo usage).
