Abstract

About 334 million people worldwide suffer from asthma, and this figure may be an underestimation. It is the most common chronic disease in children. Asthma is among the top 20 chronic conditions for global ranking of disability-adjusted life years in children; in the mid-childhood ages 5–14 years it is among the top 10 causes. Death rates from asthma in children globally range from 0.0 to 0.7 per 100,000.
There are striking global variations in the prevalence of asthma symptoms (wheeze in the past 12 months) in children, with up to 13-fold differences between countries.
Although asthma symptoms are more common in many high-income countries (HICs), some low- and middle-income countries (LMICs) also have high levels of asthma symptom prevalence.
Aim of the study:
The aim of the current study is to identify the determinants of childhood asthma and increase the awareness for house contacts of patients for the hazards of asthmatic determinants on health of their children.
Patients and methods:
It is a descriptive study carried out on children with asthma attending outpatient clinics in three medical centers in New Minia City, Minia governorate, Egypt, in the period between January and April 2019. A total of 135 children with already diagnosed asthma were included in this study, the age of the children ranged from one to ten years old.
Every child with bronchial asthma was subjected to clinical examination and anthropometric measurement. the history was taken from parents.
Results:
The study included 135 asthmatic children with mean age of 4.4±2.1 years, 77 (57%) boys and 58 (43%) girls. The average family income was 5535.5±2603.2 and ranged from 1,200 to 15,000 Egyptian pounds. Most of asthmatic children (86.7%) lived in families of more than 3 members and crowding index more than one (83.7%).
Environmental exposure was reported to occur as following: 67.4% of asthmatic children exposed to perfumes, 76.3% to detergents, 65.9% to specific foods causing asthma, 28.9% to pets, 34.8% to birds and 66.7% had no good sun exposure.
According to their caregivers, 35 % of them was born preterm, and 20 % were low birth weight 40% were admitted to NICU at birth, 78.5% were breastfed, and that 66% of breastfed children completed breastfeeding for two years.
Nearly half of the asthmatic children had family history of bronchial asthma. Skin allergy was present in 62.2% of the studied asthmatic children.
Studied children had high percentage pf reported recurrent cough at night (91.1%), 89.6% had had wheezes or crepitation, 83% experienced recurrent chest infection, 65.9% had difficulty of breath and 48.1% had exertional cough.
Asthmatic children had delayed growth, as 13% of studied asthmatic children were found to be stunted (HAZ score < -2SD), 3.7% were underweight (WAZ score < -2SD), and 8.9% were overweight or obese.
More than half of studied children were previously hospitalized due asthma, 62.7% missed at least one school day because of asthma.
Moreover, 47.8% had delayed school achievement.
