Abstract

This study was done to evaluate two different designs for treatment of mandibular bilateral free end saddle with osseointegrated implants.
10 patients were selected from the out clinic in the Prosthodontic Department in El-Minia University according to specific criteria. They had completely edentulous maxilla and Kennedy class I mandibular arch
For the maxilla, all received a complete denture. While for the mandible, Patients received two dental implants, one on each side to convert the case from free end saddle to bounded saddle. Then, patients were divided into two groups: group I: patients received extracoronal attachment (OT-Strategy) distal to the last natural abutment teeth with construction of metallic removable partial denture. group II: patients received bar attachment with construction of metallic removable partial denture.
Follow up was done at 0, 3, 6 and 9 months with evaluation of biological parameters as the attachment level and bleeding index. And radiographic evaluation for bone height and density.
On analyzing the obtained data collected through the study period, the following conclusions were obtained:
1. There was a significant difference in pocket depth between the OT-Strategy extracoronal attachment and bar attachment as the bar attachment showed higher mean values throughout the study. While there was no significant difference in the bleeding index between both groups.
2. There was no significant difference between both groups regarding bone density. While for bone height there was a significant difference on the first-time interval (0-3 m) as the bar attachment showed higher values than OT-Strategy extracoronal attachment, but there was no significant difference between them through the rest time intervals.
