Abstract

Abnormal uterine bleeding always has variable etiologies; the most common are uterine fibroids, adenomyosis, PPH and abnormally invasive placenta.By time there is an increased data in literatures about uterine artery embolization with less morbidity, mortality and hospital stay. More-over this minimally invasive procedure provide preservation of the uterus from hysterectomy with much more psychological satisfaction as well as more chances for future fertility and child bearing. After reviewing the published literatures about endovascular management of uterine lesions, most of them concluded that the complications were minor and within the accepted ranges.The present study included forty female patients with different uterine lesions, 20 patients of them were diagnosed with symptomatic fibroids, 8 patients with adenomyosis, 6 patients with abnormal placentation, 5 patients with PPH and 1 patient with AVM.Pre-embolization imaging by ultrasound and/or MRI was done to compare with the post embolization imaging follow up.In fibroid and adenomyosis groups; UAE was done by using PVA particles and there was an overall significant reduction of the total uterine,dominant fibroid and adenomyosis volumes with decrease of related symptoms and improvement of patients’ quality of life.In abnormal placentation group. There was marked decreased in amount of intra-operative and perioperative blood loss with decreased amount of fluid replacement by using occlusive balloons and Triple P procedure.In PPH group, there was complete control of bleeding by using Gelfoam particles with decreased morbidity and mortality Uterine artery embolization is an effective and one day minimally invasive procedure which showed competitive and well proved results when compared to other treatment outcomes in treating uterine lesions such as conventional surgery.It deserves the attention to its value and efficacy for the sake of the patient as a minimally invasive procedure providing better quality of life and aiming for uterine preservation and avoiding hysterectomy giving the chance for future fertility.
