Abstract
Purpose: to assess safety and efficacy of Holmium laser enbloc resection (HolERBT) versus convention transurethral resection of bladder tumor (cTURBT) Materials and Methods: 100 patients presented with non-invasive bladder cancer allocated to: cTURBT or HolERBT. The primary endpoint was the detection of residual tumor in the reTURBT specimen done 4-weeks post the primary resection. The secondary endpoints were; assessing detrusor muscle presence at the primary resection specimen, comparing the perioperative complications and the recurrence free survival. Results: Patients and tumor baseline preoperative characteristics were comparable. Residual tumors were detected in 7% and 27.7% in HolERBT and cTURBT, respectively (p=0.01). Detrusor muscle were present in 98% and 62% (p< 0.001) in HolERBT and cTURBT, respectively. cTURBT had more perioperative Clavian-Dindo complications (G1-3b) 18% versus 0% (p=0.02). HolERBT had less catheterization time (p< 0.001) and hospital stay (p= 0.001). Recurrence free survival in HolERBT were 31.76 months and 28.25 months with p=0.07. Conclusions: HolERBT is a safe procedure for bladder tumor resection. HolERBT had significantly; detrusor muscle presence in biopsy, lower residual tumor in reTURBT and better T1 sbstaging.
