Abstract

Objective: The aim of this study is to assess the efficacy of topical subgingival application of glucosamine sulphate gel 100 mg/ml adjunctive management to scaling and root planning (SRP) in treatment of patients suffering from chronic periodontitis, The proposed assessment will be based on clinical scoring and radiographic study. Subjects and Methods: The present study was carried out on 25 patients with chronic periodontitis with at least two quadrants with pocket depth >4 mm within the same arch. Split-mouth study design was used where control side received (SRP) only and study side received both (SRP) and subgingival application of glucosamine sulphate in the form of a gel prepared formula 100 mg/ml in a 6 months follow-up study. After clinical assessment and radiographic assessment by standardized digital periapical paralleling technique in all patients received full mouth SRP in both study and control sites at the baseline, 1st , 2nd, 3th and 4th weeks. Study sites additionally received glucosamine sulphate gel sub-gingivally till the depth of pocket, this done once a week starting at the baseline then repeated at weeks 1, 2, 3 and 4 for a total four applications. Standardized radiographs of the study and control sites wear taken using a paralleling technique with a film holder by a special design. Evaluation included clinical and radiographic parameters , where clinical parameters were recorded at the base line before treatment , in the 4th and then in 12th week. Clinical parameters included: Plaque index (PI), Gingival index(GI), Periodontal probing depths (PPD) and Clinical attachment loss (CAL) . Radiographs were taken prior to clinical measurements from both study and control sites at the baseline and 6th month after treatment. where, radiographic assessment was done to evaluate the additional effect of glucosamine sulphate gel 100 mg/ml to SRP. In this study Periodontal bone loss were evaluated by recording Vertical bone loss (VBL) and horizontal bone loss (HBL)at base line and 6 month later in both study and control group . Conclusions: Scaling and root planning (SRP) can result in the reduction of many clinical signs of the chronic periodontitis but applications of (100mg/ml) glucosamine sulphate gel form as an adjunctive to scaling and root planning (SRP) found to have significant clinical effects among study defects more than that achieved by (SRP) alone in controls . Glucosamine sulphate gel form has significant decreasing effects on periodontal bone loss , so glucosamine sulphate proved to be an natural ,safe and effective local delivery drug for management of chronic periodontitis.
