Abstract

Background : Mechanical ventilation (MV) is a lifesaving and support intervention but exposes patients to the risk of ventilator associated pneumonia (VAP). Chest physiotherapy (CPT) is an accepted treatment method in the intensive care unit and common preventive strategy to prevent pulmonary complications as VAP. Aim: To assess the effect of using different modalities of chest physiotherapy on prevention of ventilator associated pneumonia. Subjects and Methods: A prospective randomized clinical trial was used to conduct this study on 80 patients, aged between 18-60 years of both sexes, newly admitted to intensive care unit of emergency hospital at Mansoura university hospital from September 2015 to April 2016, data were collected using two tools; the 1st tool was multimodality CPT tool and have two parts; part one to assess socio- demographic data, part two CPT care sheet for both groups, the 2nd tool is the follow up sheet. Results: The occurrence of VAP at the seventh day of care was observed in only 15% of patients in group B versus 37.5% of patients in group A. It means that there was positive association between using different modalities of CPT and prevention of VAP in both groups. Conclusion: Using different modalities of chest physiotherapy had significant positive effect in decreasing VAP by using five types of interventions of CPT. Recommendation: Raising the awareness of VAP and how to prevent other complications from mechanical ventilator, providing nurses with continuous educational programs with evidence based guidelines to improve their knowledge and practices regarding VAP and CPT, enhancing nurses practices regarding CPT.
