Abstract

Children constitute a considerable volume of patient load in general practice. It is not uncommon to come across children with hematological disorders.
We aimed to determine the epidemiological data of pediatric hematological and malignant disorders of Menoufia pediatric patients that may provide significant data which may help in preventive, regular follow up and treatment decisions as well as to establish electronic based patient data registry system for more easy and accurate patient registry.
This study was performed on 2352 pediatric patients who had medical files at Hematology and Oncology Unit, Pediatric Department, Menoufia University Hospital from January 2015 to February 2019, after approval of medical ethics committee.
Inclusion criteria:
Pediatric patients had medical files at Hematology and Oncology Unit, Pediatric Department, Menoufia University Hospital which were collected in the period from January 2015 – February 2019.
Exclusion criteria:
1- Patients with no definite diagnosis (still under investigations for final diagnosis).
2- Patients with lost file record or with incomplete basic data.
The studied children were categorized into the following groups according to the nature of disease:
(B): Included pediatric patients with bleeding disorders (males and females). Their age ranged between 4 months and 12 years with median age 6 years.
 group (C): Included pediatric patients with malignancy (males and females). Their age ranged between 1 and 12 years with median age 6 years.
 group (D): Included pediatric patients with other hematological disorders (males and females). Their age ranged between 0.5 and 12 years with median age 6 years.
This study described the following:
 The total number of cases had medical files at hematology and oncology unit, pediatric department at Menoufia university hospitals from January 2015 to February 2019.
 The specific diagnosis of each case and their total number
 Number of patients in both sexes
 Socio-demographic characters including the age, residence and consanguinity
 The outcome of each group
Details of individual disease including the specific incidence rates were calculated and the epidemiological aspects (age, sex, residence, consanguinity) with more detailed data for individual disease according to their significance and the ranking of most frequent disease.
Results showed that:
During the period of (January 2015 – February 2019) there was 2352 patients included in this study had medical records at the unit at which 1713 (72.8%) patients had anemias (group A), 429 (18.2%) patients had bleeding disorders (group B), 165 (7%) patients attended with malignancies (group C) and 45 (1.9%) patients attended with other hematological disorders (group D).
The mean age of patients with hematological and oncological disorders in this study was (5.29 ± 4.16) years.
 There were 1548 (65.8 %) males among these patients.
 There were 1773(75.4 %) patients from rural areas.
 History of consangious marriage was present in 663 (28%) patients.
 Positive family history was present in 954 (40.6%) patients.
In this study, we report on a re-engineering process to improve medical records management in hematology and oncology unit, Pediatric Department at Menoufia university hospital. While electronic medical records management systems can improve completeness and availability of medical record information, our country lacks the needed infrastructure and resources for widespread use of electronic medical records. Therefore, we developed an electronic-based registration and medical records management system with a simple computer database program to assist patient information search.
from this study, it is concluded that:
1. The most common causes of anemia were iron deficiency anemia then G6PD deficiency then thalassemia trait.
2. The most common causes of inherited bleeding disorders were Von-Willibrand disease, haemophilia A and platelet dysfunction.
3. The most common malignancy was acute lymphocytic leukemia.
from this study it is recommended that:
1. Establishment of patient sheet for every disease.
2. Establishment of electronic based patient data registry at all hospitals in Egypt for more easy and accurate patient registry.
3. Sharing the data between the different hospitals in Egypt.
4. Sharing the data between Egypt and the other countries (universal registry).
5. Collaboration with the office of statistics and information technology to develop standards and regulations for enhancing the hospital information systems and laboratory information systems to meet the needs of registry programs and facilitate data exchange with the registries.
